The effect of nimodipine treatment was investigated in a prospective, randomized, controlled, double blind study in 45 patients with head injuries. In addition to standard treatment, nimodipine or placebo was administered in a dose of 5 mg/h for 48 h. Clinical and cognitive data (including a mini-mental state examination) were collected at admission and after 24 and 48 h. A primary outcome measure was mortality. Patients with a Glasgow Coma Scale score of 8-12 at admission had a significantly reduced mortality of 45% compared with patients with a score of 3-7 (73%). No significant differences were found between placebo and nimodipine treatment. The nimodipine group had significantly fewer patients with abnormal results on the mental status examination and fewer adverse events (nausea, vomiting and dizziness). No significant differences in laboratory values or clinical findings were found. Cerebral protection with nimodipine may not be superior to standard treatment in patients with a GCS of 8-12 after head injury.Q:
